CONSENT FORM

By executing this Consent Form and agreeing to the Terms of Service and Privacy Policy set out on the website www.impactisnow.com (“Site”), I (“I”, “applicant”) provide my consent to Impactis Global Private Limited, its holding company, affiliates, business partners, vendors, service providers, stakeholders and any other third parties at the sole discretion of Impactis Global Private Limited, and their respective officers, agents, advisors and directors, for using the contents and information globally and in perpetuity, provided by me on the Site or otherwise to Impactis for all lawful purposes including for purposes requiring my specific consent. The contents and information will include (without limitation) my photos, basic information, qualifications, information that has been filled in the application and registration process. Please note that some of this information may be personal and/or sensitive to me and accordingly I am specifically consenting for the aforesaid persons to use it as above. 

 

Further, I expressly waive and release any and all claims, liabilities and indemnities which I have or may have in the future against the aforementioned persons, in relation to the use of my contents or information by Impactis. I covenant not to make or bring any such claim against the aforementioned persons, and forever release and discharge the aforementioned persons from liability under such claims.
